Towards a five year frailty plan

We’re pleased to invite you to attend a frailty and older people event on 21 January 2021, which will be facilitated by the Acute Frailty Network.  This is a great opportunity to maximise the existing opportunities as well as the ambitions we all have  for our frail and older population. This is of significant importance to our services going forward  so your attendance is crucial, enabling us  move forward with the next phase of  the  integrated  frailty journey.

This event will build on, and be informed by, work already undertaken to develop a frailty service.

Aim

Develop an integrated, proactive whole system frailty service.

Objectives

• Developing a 5 year vision and strategy, co-designed with all system partners;
• Agree what the integrated service model should be;
• Plan the necessary capacity informed by an understanding of the population’s needs and demand
• Understand and develop the necessary skill set to deliver the agreed service model.
